Please fill out this form, Repair Request Form and email to us ahead of your shipment. You can fill it out on-line and click the "Print, Save & Send" button at the bottom of the form. Please include a copy of this form with your shipment.
We will do our best to get your equipment back to you as quickly as possible. Please do not send equipment to us without the Repair Request Form - it will result in delays if we receive repairs with no explanation and no contact person.
